TRU PACE (Program of All-Inclusive Care for the Elderly) is an innovative model of care, a managed medical plan that helps older adults meet their health care needs in their home and in the community. TRU PACE coordinates and provides needed preventive, primary, acute, and long-term care services so that older adults can continue living safely in their community.
TRU PACE will help participants and their family members navigate the complicated issues associated with aging. Going from doctor to doctor can be exhausting and take up a lot of your day. That is why many of our services are under one roof. The idea is that by having all of these services in the same place, it’s easier for participants, and they’ll also enjoy a higher standard of care. Not to mention, it takes some of the pressure off of caregivers and family members.
At the heart of TRU PACE is our Interdisciplinary Team (IDT) who will manage each participant’s plan of care. Our TRU PACE day center is where our IDT is located as well as our medical clinic, rehabilitation services, home health coordination, social work, recreational therapies, social activities, and much more. When your care team can share information about your health and progress, they can give you coordinated treatment and recommendations. TRU PACE has you covered, from head to toe. We also offer services to help you make better decisions about your health, as well help you take advantage of resources in your community.
Eligibility Requirements
TRU PACE serves older adults from all walks of life. Our program can enroll individuals who meet the following eligibility requirements:
- At least 55 years old
- Living in our TRU PACE service area
- Certified by the State of Colorado as needing nursing-home-level care, and
- Able to live safely in the community with PACE services at the time of enrollment.

TRU PACE is a health plan with a Medicare contract and is a Medicare approved Part D sponsor. TRU PACE participants must receive all needed healthcare, including primary care and special physician services (other than emergency services) from TRU PACE contracted providers. TRU PACE participants may be fully and personally liable for costs of any unauthorized out-of-network services.
TRU PACE participants may disenroll at any time.
